• SMFR - Training Request Form

    This form is your one stop shop for all things related to trainings requests; in-service, OOS, Sim Lab, Probationary Test Request. Please complete the following steps outlined below:
    1. This form can be used for OOS or In-service training that needs to be placed on the training calendar or be scheduled. 
    2. Check the SMFR - Calendar of Events to see if it is open for your desired date.
      • A maximum of 6 suppression units, any flavor, can be out of service at a time but no more of the specific unit type;
        • 5 Engines
        • 4 Medics
        • 2 Towers
        • 1 BC
      • If a unit is on the schedule but in service (pension visits), they do not count.
      • 1st day of set requests are to only be used by the shift(s) if there is nothing available on the 2nd day.
        • If your request is on the 1st day of the set, understand that this could get bumped.  1st day of the set is reserved for SMFR specific.
      • Training requests cannot be submitted less than 106 hours prior to the training.  For example, Cannot schedule after 1900 on the last day of the set for the 1st day of the following set.
      • If below the allowable 'out of service' limits after counting your unit(s), AND the desired day is not on the 1st day of the set, move to the next step.

    3. Contact your BC and get their permission to go OOS. Give them an idea of what you are planning on doing and how long you will be out of service
    4. If the Training is held in the EMS - SIM Lab, obtain approval from the Training.EMS  EMSTraining@SMFRA.onmicrosoft.com prior to submitting this request.
    5. After Completing each step above, then click the 'Next' button below

  • This Session CANNOT  be scheduled because training cannot be requested after 1900 on the 2nd day of the current shift for training on the 1st day of the following set.

    If your BC made the request, then it will be discussed during the morning BC conference call.

     

  •  Checking "Denver Fire TJ Request" is only for Denver Fire.  Southmetro employees must pick a shift.  Please select the correct shift.

  • Training Session Location & Availability

  • Reminder:

    ~A maximum of 5 Engines, 4 Medics, 1 Tower, and 1 BC can be out of service at any one time. If a unit is on the schedule but in service (pension visits), they do not count.

    ~No Training can be scheduled if 1st day of the set.  Get approval from ISO's prior to scheduling and make a note of it in your request.

  • When documenting Objectives: Please document all the skills and tasks you would like to complete & the location(s) on the drill ground you would like to use.
